The lung transplant is an interesting question. How many of us have living will directives that would contain that degree of specificity? If I were to be precipitously struck with the same scenario as GuyF it would likely not occur to my substitute decision-maker(s) to investigate lung replacement, though I have specified in my advance directives some general references to coma duration endpoint and judgement of futility, etc.
 
Are ICU’s covering off, initiating, transplantation option considerations in consultation with surrogate decision-makers? I suspect not, given the volume of acutely ill COVID patients, and the appointed person would likely have to assertively come at it with the notion that everything short of cryogenics is on the table.

It’s VERY common for covid patients in ventilators to not be able to be weaned. Such patients are not candidates for transplants. They are far too ill.

Not trying to micromanage but a busy Social Worker with now an appointed decision-maker may not have time to keep @Cooper in the loop and neither the SW nor the appointee know all of our MO context. I don't see it that way. I would think the DNR relieves the EMRs from having to make the decision since the decision is made before hand by the person who wrote it.

It’s so hard for me to believe the Gaiety has now been gone for 16 years. How is that possible? So many fun filled memories. Aside from the obvious pleasure of watching the show and meeting the dancers I’m happy to report that Cooper and I bonded and have remained good friends all these years. Except for the pandemic we continued meeting in the city almost every Wednesday to take in a Broadway show, have lunch or dinner and continue on with cocktails. For years that was Splash Bar and when that closed (very sadly) we moved on to other bars. In the last few years that has been Hardware. Even though we haven’t been able to see each other in person this last year we text almost every day. I‘m looking forward to resuming our weekly meetings.
